Change my address	Complete an online form or Visit one of our stores	If we are unable to verify your previous address with our records, or you do not know your previous address, you must visit a store to update your information.
Request a new Hbc Rewards Card	Visit one of our stores	You can continue to use your old Hbc Rewards card. However, if you wish to have a new card please visit any of the Hbc Family of stores to obtain your new Hbc Rewards Card. A store associate will be able to reinstate your points balance on the new card. Your request can not be processed online.
Enroll as a new Hbc Rewards member	Complete an online form or Visit one of our stores	Become a member of Hbc Rewards to begin collecting points.
Reprint my Hbc Rewards Card	Visit one of our stores	ID must be presented and verified.
Get a supplementary Hbc Rewards Card.	Visit one of our stores	The primary cardholder must visit a store to request a supplementary Hbc Rewards Card.
Change of Community Address	Complete an online form	If your Community account information changes at any time, please make the changes online.

AIR MILES^® Rewards Miles

As an Hbc Rewards Collector, you have the option to automatically redeem your Hbc Rewards points for AIR MILES^® rewards miles every week. Hbc Rewards points will be redeemed at a rate of 2,000 points to 1 AIR MILES^® reward mile. These AIR MILES^® reward miles will be displayed on your AIR MILES^® reward miles Summary Statement, starting October 2001.

Acceptance of Terms and Conditions

By signing this enrollment form and/or by using your Hbc Rewards card for the first time, you (i) certify that you are over the age of majority in the province in which you reside (or you have obtained the consent of your parent or legal guardian), (ii) consent to our collection, use and disclosure of your personal information except as you may otherwise notify us, as set out above, and (iii) agree to the terms and conditions of the Hbc Rewards program, as they may be amended from time to time. The current terms and conditions of the Hbc Rewards program are set out in the Hbc Rewards catalogue and online at www.hbc.com/rewards. Copies of the terms and conditions are also available in-store at the Bay, Zellers and Home Outfitters. You may also request that a copy of the terms and conditions be mailed to you by contacting us as set out above.
